Please Note: If Provider is not in the 3 networks provided, please call the Connect Concierge for assistance. They can work with ELAP to get your provider to accept the plan. We have close to a 99% acceptance, nationally.

For Inpatient Care including in-patient procedures and surgeries, you must contact the Connect Concierge at 855-438-2011.

They will assist you in pre-certification for the procedure or surgery, help you find physicians and facilities in the EHN network, ELAP, or through the SurgeryPlus Bundled Pricing Program. The Connect Concierge will help minimize your out-of-pocket costs and maximize your choices by assisting with hospital and facility selection. You will also receive personalized support throughout your care journey.
